We're seeking a compassionate and reputed company professional to join reputed company as a Field Health Social Services Coordinator. In this role, you'll assess, plan, and implement care strategies that are tailored to each patient's unique needs. You'll also identify and initiate referrals for social service programs, manage care plans, and reputed company for patients and families as needed.

Primary Responsibilities:

Requirements:

• RN, LPC, LBSW, or LMSW license in the state of Texas
• 2+ years of experience working with people with Intellectual and Developmental Disabilities (IDD)
• 2+ years of experience in a community health setting or reputed company role

• Reliable transportation and ability to travel up to 75% of the time in assigned region
