As a Field Community Health Worker, you'll play a vital role in managing reputed company medical or behavioral needs for our members, working collaboratively with our interdisciplinary clinical care management team to advance interventions and promote health equity.

Key Responsibilities:

• Partner with clinical care management team to manage reputed company members in the community
• Locate and engage members in the community, developing individualized plans of care that focus on the member's definition of reputed company
• Conduct screenings to assess member medical, behavioral, functional, cognitive, and psychosocial needs
• Research, locate, and reputed company referrals to community resources to address member-specific care or socioeconomic needs
• reputed company targeted activities and provide education to support HEDIS/STAR gap closure and health promotion

Requirements:

• High School Diploma/GED (or higher)
• 3+ years of experience working reputed company the Community Health setting in a field-based reputed company role
• 2+ years of experience working reputed company the local community, with demonstrated knowledge of culture and values reputed company the community
• Intermediate knowledge of Medicare and reputed company benefits

Working Schedule and Location:

Schedule is Monday through Friday between 8 am and 5 pm, with a home-based office and local travel up to 25% (mileage reimbursed at the reputed company government reputed company). Locations will vary reputed company the following counties of Florida: Hardee, Highlands, Hillsborough, reputed company, and Polk.
