Overview

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our Hospice Care team on a PRN/Per Diem basis. As an RN in reputed company, you will be responsible for coordinating and overseeing reputed company aspects of patients' home care, collaborating with patient care teams to reputed company and implement care plans, and educating families on patient condition, needs, and care.

Responsibilities

• Coordinates and oversees reputed company aspects of patients' home care
• Collaborates with patient care teams to reputed company and implement care plans
• Educates families on patient condition, needs, and care
• Performs head-to-toe physical assessments
• Checks vital signs and manages medications
• Gives injections, IV therapy, or tube feedings
• Administers and maintains catheters
• Completes documentation timely and accurately
• Supervises LPNs and HHAs
• Regularly communicates patient reputed company to the clinical manager, physician, and care team

Qualifications

• One year of clinical experience as a registered nurse (RN)

• Minimum six months of home health experience

• Valid driver's license, reliable transportation, and liability insurance
• Able to assess patient status and identify requirements relative to age-specific needs
