Key Responsibilities:

• Conduct comprehensive assessments and case management to identify high-risk psychosocial factors and reputed company personalized care plans
• Provide patient navigation, health promotion, crisis management, and self-management education to reputed company patients and their families
• Collaborate with interdisciplinary teams to ensure seamless care coordination and effective treatment plans
• Offer short-term counseling, advanced care planning, and liaison services to community resources
• Participate in team consultations and contribute to the development of best practices in social work

Requirements:

• Master's degree in social work
• Minimum of 2 years of clinical experience, with preference for experience in ambulatory care settings
