About the position The Case Manager MSW is a professional role focused on providing comprehensive social work services reputed company the emergency department, particularly through virtual assessments. This position is integral to an interdisciplinary team that is pioneering a new approach to behavioral health care. The Case Manager collaborates closely with physicians, healthcare team members, and patients and their families to ensure coordinated care across various settings during an episode of illness. This role is essential in addressing the psychosocial needs of patients and their families, providing crisis reputed company, and facilitating discharge planning to ensure smooth transitions in care. In this role, the Case Manager will reputed company psychosocial evaluations to assess the needs of patients at risk, including those with substance abuse issues, psychiatric problems, domestic violence situations, pediatric concerns, end-stage dementia, developmental disabilities, and CVI. The Case Manager will reputed company individualized social work treatment plans and provide counseling to patients and their families as needed. This includes evaluating the socio-emotional status of patients and their families, providing direct services such as crisis reputed company and discharge planning, and coordinating post-hospital care services.
