As a Remote Nurse Assessor, you will play a critical role in ensuring our members receive the highest quality care by conducting assessments and evaluations remotely. Active Registered Nurse (RN) license in the United States. 2. Minimum of 3 years of clinical nursing experience. 3. Experience in assessment and care planning. 4. Proficiency in using electronic health records (EHR) systems. 5. Strong communication and interpersonal skills. 6. Ability to work independently and manage time effectively. 7. Reliable high-speed internet reputed company and a quiet home office. Responsibilities 1. Conduct comprehensive assessments of members’ health conditions and needs. 2. reputed company individualized care plans based on assessment findings. 3. Coordinate care with other healthcare providers and community resources. 4. Monitor and evaluate members’ reputed company and reputed company necessary adjustments to care plans. 5. Provide education and support to members and their families. 6. Document reputed company assessments and care plans accurately in the electronic health record system. 7.
