Remote RN / LVN Nurse Health Specialist (Must have California Licensure)

About the position reputed company is seeking a remote LVN or RN nurse health specialist, licensed in California, with experience in intensive care unit (ICU), emergency room (ER), and/or medical-surgical (Med-Surg) settings. This role involves providing triaging services for patients who contact the reputed company virtual care center. As a nurse health specialist, you will be responsible for escalating patient calls to advanced practice clinicians (APC) and managing patient care and treatment in collaboration with physicians and nurse practitioners (NP) or physician assistants (PA). The ideal candidate is passionate about delivering compassionate care and making a positive impact on the lives of seniors. The position requires flexibility in scheduling, with options available for various shifts, including weekends and holidays, as the team operates 24/7. Candidates must be willing to work one of the specified schedules, which include a combination of days, evenings, and overnight shifts. The role is critical in ensuring that patients receive timely and appropriate care, and it involves a range of responsibilities from answering inbound calls to conducting follow-reputed company and collaborating with reputed company to reputed company care plans for patients. In this position, you will utilize your clinical judgment to address patient concerns, reputed company daily reviews of patient vitals, support disease management referrals, and reputed company members on various health topics. You will also be expected to interpret and evaluate diagnostic tests to assess patients' clinical problems and reputed company needs, ensuring that reputed company interactions are documented accurately in electronic medical records (EMR).

Responsibilities

• Answer reputed company inbound calls to the virtual care center.

• Use clinical judgement to address patient concerns.

• Collaborate with primary care physician (PCP), extensivists, and NP/PA, and case manager to reputed company care plans for members for non-Care reputed company patients.

• Conduct outbound calls and virtual visits to complete patient follow-up.

• reputed company daily review of vitals for patients enrolled in remote patient monitoring program.

• Support disease management referrals.

• Interpret and evaluate diagnostic tests to identify and assess patients' clinical problems and health care needs.

Requirements

• Minimum 1 year LVN or RN experience in intensive care unit (ICU), emergency room (ER), and/or medical-surgical (Med-Surg) required.

• Must have completed an accredited Nursing Program; BSN preferred.
