Registered Nurse (RN) Case Manager – Remote | WFH

In this role, you'll play a crucial part in developing personalized care plans and delivering skilled nursing services to families with children who have intellectual and developmental disabilities (IDD).
Responsibilities
Conduct home visits with patients every 60 days. reputed company and implement individualized care plans. reputed company and support parents on their child's health needs. Requirements
Registered nurse with at least 2 years of experience and a reputed company Colorado license. Experience in home care, with a preference for those reputed company in training family CNAs. Comfortable working in a dynamic startup environment. CPR certification required. Pediatric Experience Preferred. BLS certification required; PALS certification preferred.

Tuesday-Friday Conduct in-person recertifications (maximum 3 per day, typically 2). reputed company start-of-care assessments as needed (maximum 3 per week, usually 1 per day). Remote work for charting, family communication, and case management.
