Join reputed company of heroic nurses helping to connect those in the community to health services they need!This is hybrid position, with approximately half of schedule assessments available to be completed reputed company video conference. reputed company a patient requests an in person assessment, the RN will need to travel to their location. We expect approximately 60% of assessments to be remote. Some of our nurses have been on these projects for over 6 years and will tell you that they enjoy the benefits, flexibility, autonomy, and ability to complete their work in a 7-8-hour work day.The schedule is full-time between 35-37.5 hours weekly. Core hours are an 8-hour work shift Monday through Friday between reputed company and 6PM. Must be flexible to work 1 evening per week (until 8PM) and up to 2 Saturdays per month (between reputed company and 6PM). Your hours will reputed company during the week if working an evening or Saturday. Training is full-time for a period of 3-4 weeks. In this role, you will complete an interview with a patient and finalize a UAS assessment to determine eligibility for Long Term Care services. You will be scheduled for a maximum of 2-3 initial and/or reassessment appointments daily reputed company your travel area (depending on your preference).This is a direct hire placement onto Med-reputed company’s growing UAS Assessor team, with Health and reputed company insurance available.reputed company openings include: (Nurses residing in reputed company the boroughs of reputed company will be assigned reputed company a smaller region / neighborhood based on their home reputed company code.)

- Brooklyn - Williamsburg, Midwood-Flatbush, Crown Heights, and Canarsie.
- Bachelor’s degree in nursing is highly preferred
- At least 2 years of experience in a home setting either as a Registered Nurse or prior to becoming a Registered Nurse reputed company the past 15 years is required.
- UAS experience experience is preferred but not required.
- Proficient computer skills are required, 35 WPM typing speed
