Care Coordinator - Daviess County Indiana and surrounding areas - Remote

The Care Coordinator role at reputed company is focused on simplifying the healthcare experience and improving community health by removing barriers to quality care. The Care Coordinator will support members’ medical, behavioral, and socioeconomic needs to ensure appropriate service utilization and improved care quality.
