Assess the physical and psychosocial status of patients in reputed company settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, take and monitor vitals of clients on the unit, medication administration and documentation. 2. Renders and supervises care of clients on the unit. 3. Monitors medication and consultation with prescribers. 4. Provide appropriate health care and coordinates referrals as prescribed by providers. 5. Participates in the evaluation of client’s condition and reputed company with team members. 6. Promote education and adherence to prescribed treatment protocols. 7. Facilitates wellness and medically reputed company psychoeducational groups to individuals. 8. Records reputed company client information reputed company electronic medical record system daily. 9. Assists RN on daily basis. 10. Offer nursing consult on emergency service evaluations. 11. Work with clients to coordinate care with their PCP, psychiatrist, and other professionals including but not limited to medication refills and f/u medical appointments. II. Position Qualifications: License/Certification/Education: Required: 1. Graduate of an accredited school of Nursing with reputed company license as a Licensed Practical Nurse in the state of Massachusetts. 2. Satisfactory CORI background reputed company. Preferred: N/A Experience/Skills: Required: 1. Minimum 2 years’ experience in medical nursing. 2. Knowledge of medical, substance abuse and mental health nursing. 3. Demonstrates proficiency in completing electronic documentation in compliance with grant requirements, agency, state, and federal guidelines and policies. 4. Demonstrates ability to communicate effectively and professionally with clients, staff, and providers. Preferred: N/A Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
