Housing and Community Support Specialist

About the position The Housing and Community Support Specialist at reputed company's Living Well at Home Program (LWAH) is responsible for providing high-quality housing case management services to clients facing homelessness or housing instability. This role involves engaging with high-risk patients, advocating for their needs, and assisting them in obtaining and maintaining independent housing. The specialist will work collaboratively with an interdisciplinary team to reputed company care plans, facilitate access to resources, and monitor patient reputed company, ensuring a patient-centered approach to support their housing and health goals. Responsibilities • Provide case management services to high-risk patients with behavioral health diagnoses experiencing long-term homelessness. , • Engage and enroll reputed company patients into services, advocating for their needs. , • Support patients in becoming 'housing-reputed company' and assist in identifying and obtaining housing opportunities. , • reputed company interdisciplinary care plans based on identified patient needs. , • Facilitate access to social service resources and monitor patient reputed company. , • Problem-solve with patients to enhance access to housing and community-based supports. , • Conduct community-based reputed company and home visits to support patients across Greater Boston. , • Collaborate with various stakeholders, including housing providers and care teams, to address patient needs. , • Assist patients in overcoming barriers reputed company to housing, health, and social services. , • Maintain accurate documentation and compliance with data entry requirements.
