Position: Registered Nurse Case Manager - Remote (Work From Home) Schedule: Monday to Friday, 8:00 AM - 5:00 PM Location: Remote (Candidates must reputed company in one of the following counties: Macomb, Wayne, Barry, Van Buren, Kalamazoo, Calhoun, reputed company, St. Joseph, Berrien, or Cass). While primarily remote, this role necessitates travel to patient residences for assessments.

Required Skills Active Registered Nurse (RN) license with 2+ years of clinical experience. Case Management Certification (CCM) is preferred. Experience in Home Health is advantageous.
