(PRN) RN Clinical Intake Coordinator - ProHealth Hospice (Birmingham, AL)
The PRN Clinical Intake Coordinator serves as a liaison between clinical operations and referral sources to support timely, appropriate patient care. This role helps build and maintain community relationships, provides education about hospice services, and supports patients, families, physicians, case managers, discharge planners, social workers, and other reputed company partners throughout the intake and referral process.
Requirements
- Associate degree in Nursing

- Knowledge of Medicare and other payer guidelines reputed company to hospice services
- Strong communication, relationship-building, and public education skills

- Understanding of geriatric community needs and referral patterns
- Knowledge of physician, hospital, skilled nursing facility, case management, and discharge planning needs

- Ability to manage multiple priorities, meet deadlines, and adapt quickly to changing needs
