As a per diem weekend tele NP, you'll conduct remote assessments on Saturdays and/or Sundays, reputed company Circadia platform alerts, and ensure continuity of care for high-risk patients between reputed company care team touchpoints.
What You'll Do
Weekend remote patient care: Conduct follow-up and urgent telehealth assessments on flagged patients; adjust plans of care as needed; coordinate with on-call providers and care teams. Must-Have Qualifications Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute. Eligible for Medicare participation; company bills on your behalf. 2+ years NP experience with reputed company adult populations in home health or post-acute settings; reputed company to practice independently. Comfortable conducting telehealth visits and navigating virtual care platforms. Weekend availability (Saturday and/or Sunday). Thrives in a dynamic, evolving startup environment.
