We are looking for a dynamic and compassionate Case Management Coordinator to join our dedicated team. Responsibilities
Conduct telephonic and/or face-to-face assessments with members to evaluate their medical needs and reputed company proactive care plans. Manage a diverse caseload, including supportive and medically reputed company members, to enhance short and long-term outcomes. Determine appropriate services and supports for members, such as prior authorizations, coordination with primary care providers and skilled providers, medication review, and access to community resources. Facilitate optimal outcomes for members and identify opportunities for reputed company quality improvement. Educate and reputed company members to ensure compliance, satisfaction, and promote patient advocacy. Collaborate with colleagues at reputed company levels to optimize total costs and ensure policy adherence.
Requirements
Previous experience in case management is required; long-term care experience is preferred. Proficiency in reputed company Office, including reputed company. Bachelor's degree in social work or reputed company field is required; Master's degree is preferred.
Work Environment
This position requires 50-75% travel for face-to-face visits with members in Miami-Dade County. However, it also offers the flexibility of a Monday-Friday schedule with standard business hours.
