The Transitions of Care (TOC) Manager is responsible for designing, implementing, and overseeing pharmacy services that support safe, effective, and seamless transitions across the continuum of care. This role leads clinical programs aimed at reducing medication-related readmissions, optimizing medication therapy, improving care coordination, and enhancing patient outcomes during admission, discharge, and post-discharge follow-up. The TOC Manager serves as a clinical expert, operational leader, and liaison between inpatient, outpatient, and ambulatory pharmacy teams.   • Develops workflows, protocols, and policies that support efficient and compliant TOC operations.
  • Collaborates with community pharmacies to ensure timely prescription processing, delivery, and financial assistance coordination.
  • Provides leadership and supervision to designated groups of employees including assurance of quality service delivered by a competent staff to consistently meet the needs of the hospital patients.

  • Provides supervision and reporting structure to assure productivity and performance.
  • Ensures clear role expectations for staff, including expectation documents and verbal communications.
  • Coordinates individual and group training along with effective cross-training programs.
  • Ensures compliance of all departmental staff with mandatory education and employee health.
  • Identifies opportunities for staff professional development/continuing education/in-service education.
  • Promotes a sense of organizational ownership within the team for managing change and achieving organizational goals.
  • Develop, implement, and continuously improve TOC pharmacy programs aligned with organizational goals and regulatory requirements.
  • Lead multidisciplinary initiatives to enhance medication safety, reconciliation accuracy, and discharge medication processes.
  • Establish performance metrics, monitor program outcomes, and report results to pharmacy and hospital leadership.
  • Identify gaps in the medication-use process during transitions and implement evidence-based quality improvement initiatives.
  • Oversee or directly perform medication reconciliation at admission, transfer, and discharge for high-risk patients.
  • Conduct comprehensive medication reviews and optimize pharmacotherapy using clinical guidelines and patient-specific factors.
  • Provide patient and caregiver counseling on discharge medications, including adherence support and medication-use education.
  • Coordinate post-discharge follow-up, including medication access, MTM visits, collaborative practice services, and refill management.
  • Establishes effective personal relationships with medical and nursing leaders involved with drug use policy and safety while building essential team relationships among pharmacists, nurses, and physicians.
  • Responsible for supervising and performing all pharmacy operations in accordance with hospital policies and procedures, ethical and professional practices, accreditation standards, State and Federal requirements and National Patient Safety Goals regarding medication management.

Minimum Requirements

  • Bachelor's degree in pharmacy, or Doctorate in Pharmacy required.
  • Minimum of 3 years of job-related experience.
  • PGY-1/PGY-2 ASHP Residency in administration.
  • Management experience required.
  • Proficiency with Microsoft Excel, Microsoft Word, Microsoft PowerPoint and other pharmacy related applications (Omnicell, Epic) preferred.
  • Current Kentucky and Indiana Pharmacist License or eligible.
  • Must obtain Kentucky and Indiana Pharmacist License within 1 year.
