Job Summary

We're seeking an experienced Case Management Specialist to join our remote team, working from the comfort of your own home. As a key member of our team, you'll play a vital role in delivering exceptional patient care and services to our dual-eligible population. With a strong focus on case management, you'll work closely with healthcare providers, community organizations, and patients to ensure seamless care coordination and optimal health outcomes.

Key Responsibilities

* Conduct comprehensive assessments of patients' needs and develop personalized care plans to achieve optimal health outcomes

  • Identify high-risk factors and service needs that may impact patient outcomes and collaborate with healthcare providers to address these issues
  • Coordinate and implement care plan activities, monitoring progress and making adjustments as needed
  • Collaborate with case managers, healthcare providers, and other stakeholders to ensure seamless care coordination and optimal health outcomes
  • Identify and address quality of care issues through established channels
  • Utilize effective communication and interpersonal skills to engage patients and their families in the care planning process
  • Provide education, support, and resources to empower patients to make informed decisions about their health and well-being
  • Facilitate patient engagement and empowerment through effective communication and collaboration with healthcare providers and community organizations

Essential Qualifications

* 3+ years of experience in a healthcare-related field

  • Bachelor's degree or equivalent experience required
  • Strong understanding of case management principles and practices
  • Excellent communication and interpersonal skills
  • Ability to work independently and collaboratively as part of a remote team
  • Strong analytical and problem-solving skills
  • Proficiency in electronic health records and other relevant software systems

Preferred Qualifications

* Certification in case management (CRC, CDMS, CRRN, COHN, or CCM)

  • Experience working with geriatric populations, social health, and disability populations
  • Bilingual or multilingual skills (English/Spanish, English/Creole)
  • Experience with evaluation, screenings, and care planning
  • Strong knowledge of healthcare regulations and policies

Compensation and Benefits

* Competitive salary range: $19.52 - $40.90 per hour (base hourly rate or base yearly full-time compensation)
