Job Summary

As a Case Manager at arenaflex, you will play a critical role in ensuring that our customers receive the best possible care and services. You will be responsible for assessing individual needs, developing and implementing care plans, and providing support and guidance to our customers. This is a remote data entry position that requires strong communication and organizational skills, as well as the ability to work independently.

Key Responsibilities

* Conduct thorough assessments of individual needs and develop care plans that address their specific requirements

  • Work with healthcare providers and other stakeholders to ensure that our customers receive the best possible care and services
  • Facilitate and implement care plan activities and monitor progress
  • Identify and address high-risk factors and service needs that may impact customer outcomes
  • Collaborate with multidisciplinary teams to achieve optimal results
  • Recognize and address quality of care issues through established channels
  • Use effective communication and influencing skills to secure suitable choices and services for our customers
  • Provide training, information, and support to empower our customers to make informed decisions about their healthcare and well-being
  • Help our customers effectively and knowledgeably participate in healthcare decision-making

Essential Qualifications

* Bachelor's degree in a related field, such as behavioral health or human services

  • 2+ years of experience in conduct health or social services
  • Strong knowledge of Microsoft Office applications, including Word, Excel, and Outlook
  • Excellent communication and organizational skills
  • Ability to work independently and as part of a team

Preferred Qualifications

* Experience in case management and care planning

  • Supervisory experience in healthcare or social services
  • Education and certification in a related field, such as psychology, social work, or marriage and family therapy

  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong organizational and time management skills
