This role focuses on providing in-home post-discharge care management and medical treatment for patients transitioning from acute care facilities back to their homes.

Key Responsibilities

• Conduct in-home patient visits to assess clinical status and provide post-discharge medical care.
• Review and interpret discharge summaries and reputed company notes.
• Collaborate closely with home health professionals and other members of the care team to ensure seamless coordination of care.
• Provide medical consultations, prescribe medications reputed company appropriate, and reputed company ongoing care plans to promote recovery and reduce readmissions.

Qualifications

Compensation:
