This role provides advanced practice care to patients in home-based settings and skilled nursing facilities. The position is Monday through Friday with a shared on-call rotation. A minimum of 3–4 years of experience as a practicing Nurse Practitioner is required. Prior leadership experience in nursing or advanced practice nursing is strongly preferred.
