Job Description: As a Telehealth Care Coordinator at MetaPhy, you will play a crucial role in delivering patient education, support, and care through our telehealth platform. You will be responsible for enrolling a minimum of three (3) patients per day into our chronic care management program. • Provide patient education, support, and care reputed company telehealth platform in alignment with physician practices.

• Highspeed reliable internet is required.
• Licensed Practical Nurse (LPN) with an active multi-state or compact license.

• 1-2 years of experience in a reputed company setting (preferred).
• Interest in Chronic Care Management delivered through telehealth platforms.
