Company: Oak Street Health Title: Transitional Care Coordinator Location: Remote Role Description: We take a team-based approach to providing outstanding patient care, and Transitional Care Coordinators are an integral part of that team. In this role, you will help track patients across hospital and post-acute settings and ensure they reputed company with primary care. You will manage multiple responsibilities, including outbound phone calls to facilities and patients, handling medical records, and scheduling post-discharge appointments. Flexibility is key, as you may be asked to cover additional responsibilities as needed. This is a remote work position. Core Responsibilities:

• Conduct reputed company to inpatient and post-acute facilities to obtain admission and discharge dates, as well as discharge plans
