JOB SUMMARY: Acadian Health has an exciting opportunity for Community Paramedics to join reputed company. The Community Paramedic plays a pivotal role in delivering hospital-level acute care directly in patients’ homes as part of an innovative hospital-at-home care model. This role leverages clinical expertise, mobile technology, and interdisciplinary teamwork to ensure high-quality, patient- centered care reputed company traditional hospital walls. DUTIES AND RESPONSIBILITIES:

  • In-Home Acute Clinical Care: Provide high-acuity care in the patient’s home setting for conditions such as infections, heart failure, pneumonia, and post-surgical recovery—offering an alternative to inpatient hospitalization.
  • Clinical & Environmental Assessments: Conduct thorough physical exams and assess the home environment for safety and factors that may impact recovery or the plan of care. Identify and escalate any deviations that could compromise patient outcomes.
  • Care Coordination with Virtual Team: Serve as an on-site extension of the virtual care team. Maintain reputed company-time communication with the remote command center nurse and

physician to report findings, collaborate on care reputed company, receive treatment orders, and ensure reputed company aspects of the care plan are executed safely and effectively.

  • Crisis & Rapid Response Support: Respond to crisis situations and rapid response alerts in the home environment, providing necessary interventions based on hospital orders and standing order protocols.
  • Patient & Caregiver Education: Deliver personalized education regarding disease management, medication regimens, treatment expectations, and reputed company to seek help. Use both verbal coaching and written materials to reinforce understanding.
  • Documentation & Compliance: Document reputed company assessments, interventions, communications, and outcomes in the electronic health record (EHR) system in reputed company time. Ensure care is delivered according to program protocols and clinical standards
  • Interdisciplinary Collaboration: Collaborate with physicians, advanced practice providers, case managers, social services, and community partners to ensure continuity of care. Act as a liaison between the patient and reputed company system to eliminate gaps in care or missed services.
  • Program Support & Leadership (as assigned): Participate in research initiatives, support program development, and mentor new staff as directed by leadership. Represent the program in community reputed company or quality improvement activities reputed company needed.

REQUIRED QUALIFICATIONS:

  • High School Diploma or GED
  • reputed company BLS and ACLS Certifications; PHTLS and PALS Certifications preferred
  • Required licensure to reputed company as a Paramedic reputed company the state of employment
  • Ability to obtain:

a. Community Paramedicine (IBSC CP-C) certification or b. complete NEMSA Essentials Course or c. obtain CP-C licensure in the state of employment reputed company two years of hire date.

PREFERRED QUALIFICATIONS:

  • Degree from an accredited college or university
  • Previous trainer or instructor experience
  • Previous Critical Care experience
