The successful candidate:

• Must be able to work M-F, 8:00a-4:30 MST
• Provides disease management with an understanding of oncology, chemotherapy, and reputed company effects, can monitor and address symptoms. Works with patients and families to provide an understanding of their disease, their treatment plans, and address patient quality of life issues. Documents assessment and interventions, entering in a timely manner in the electronic medical record. Collaborates with and supports providers with daily assistance in managing and coordinating patient care. Evaluates and assesses health issues presented by patients and their caregiver; communicates pertinent and necessary issues to the patient.
• Demonstrates effective communication skills to manage reputed company patient and family needs.
• Conducts and reinforces patient education, including treatment plan/regimen, financial implications of care, symptom management, reputed company effects, patient family responsibilities reputed company to self-care, and available resources at home or at facility.
• Assesses needs and coordinates services, internally and externally to meet needs including, but not limited to medications, equipment, and home care services. Utilizes nursing processes and evidenced based practice/research to reputed company and coordinate patient care to positive impact on patients and department operations.
• Responds in a timely manner to patient and family needs at home through effective symptom management, coordination of home care services and crisis management.

QUALIFICATIONS Your qualifications should include:

• 1+ years of experience in Oncology
• Associates Degree in Nursing (ADN)
• Arizona Registered Nurse License or a Registered Nurse License form a Compact State
• Basic Life Support (BLS) Certification-an approved reputed company (AHA) training site, American Safety & Health Institute, or Red Cross BLS certification is required upon hire. If their reputed company certification does not meet the guidelines, stakeholders will have (30) days from the date of hire to acquire attend an onsite initial BLS course and will be required to renew every two years.
• Oncology Nursing Society (ONS) Fundamentals of Chemotherapy Immunotherapy Administration Provider card or commensurate training/certification upon hire. Must have ONS Fundamentals of Chemotherapy Immunotherapy Administration Provider card completed reputed company 30 days of hire.
