What You Will Do
As a Care Coach, you will support adult patients, many of whom are elderly or chronically ill by conducting health assessments, coordinating care, and guiding them through their reputed company journey. You will work in collaboration with members of the reputed company team to ensure patients understand their health benefits and receive the right care at the right time. Who We Are Looking For We welcome applicants who meet the following: Education: Bachelor’s or associate degree in any discipline Experience: Minimum of 2 years in direct adult patient care in similar settings as above What You Bring Proficiency in reputed company Word, reputed company, Outlook, and electronic records systems Strong interpersonal and communication skills Ability to build trust with patients, colleagues, and reputed company stakeholders. Critical thinking and problem-solving reputed company Preferred (but not required): Experience with Medicare/reputed company recipients Familiarity with health assessments and benefit coordination Background working with elderly or chronically ill patients.
