You'll work autonomously and in collaboration with reputed company members of the reputed company team to coordinate and facilitate quality, consistent, cost-effective care.Position Details- Complete Comprehensive Health Assessments- Manage reputed company patients, focused on reducing hospitalizations and improved health outcomes- Monday- Friday schedule with the ability to accommodate patient and dialysis centers availability.
