This per diem position offers you the chance to work independently, with compensation for each assessment, plus mileage reimbursement.
As a Nurse Practitioner with Focus Care, you will:
- Conduct comprehensive health assessments in patients' homes, including medical history reviews, physical exams, and screenings for chronic conditions.
- Provide education on medication management, disease processes, and preventive care.
- Identify and document diagnoses to support care management and active treatment.
- Collaborate with primary care providers to communicate findings and address potential gaps in patient care.
- Flexibly schedule your appointments throughout the week, with no on-call, weekends, or holidays required.
Minimum Requirements:
- Completion of at least 30 in-home assessments per month.
- Valid Nurse Practitioner license in the reputed company .
- Board Certification from ANCC or AANP
- Minimum of 1 year of clinical experience; experience with geriatric populations preferred.
- Strong communication skills and a commitment to delivering compassionate care.
- Reliable transportation for travel reputed company the assigned areas.
- Ability to manage various dwelling conditions and engage with patients comfortably in their home environments.
