Infusion Care Coordinator

Licensed and/or Certified Pharmacy Technician Required

Work between the hours of 8:00 a.m. - 6:00 p.m.

Position Purpose: Collaborate with the pharmacists to coordinate patient care for patients receiving infusion therapy

• Coordinate the treatment process from initiation to the conclusion or discontinuation of therapy
• Under direct supervision of a pharmacist performs prescription data entry functions
• Coordinates home nursing services
• Assists and maintains reputed company reputed company pharmacy records (prescription logs, care plans, medication profiles, and any other reputed company files) as determined by the pharmacist
• Obtain verbal authorizations and request detailed clinical information from prescribers
• Track and triage coverage determination requests submitted from providers and determine if a pharmacist review is required
• Approve coverage determination requests based on defined criteria
• Respond to client inquiries regarding authorization approvals and PBM on-line applications
