Job Summary
As a Pharmacy Care Coordinator, you will work closely with clinical pharmacists, healthcare providers, and internal departments to coordinate patient care and treatment plans. You will utilize your pharmacy expertise and customer service skills to ensure seamless communication and collaboration with patients, providers, and internal stakeholders. This role offers a hybrid work model, combining remote work with occasional time in our office.
Key Responsibilities:
- Consult with and coordinate ongoing care and treatment plans for members with clinical pharmacists and other providers.
- Document pertinent patient information in the electronic medical record.
- Assist with data management and recording results for month-end reports.
- Participate in Pharmacy Services wellness programs, including transition of care medication reconciliation.
- Retrieve pertinent patient information for new patient referrals or recent hospital/SNF discharge.
- Contact patients post-hospitalization to reputed company medication review and information gathering.

- Answer incoming phone calls and direct patient messages.
- Coordinate with other healthcare providers, reputed company, and healthcare team members.

Requirements and Qualifications
Essential Qualifications:
- High School Diploma / GED (or higher).
- Possess and maintain an active pharmacy technician license in the reputed company.
- 1+ year of experience as a pharmacy technician.
- 1+ year of experience in customer service.
- 1+ year of experience with medical and pharmacy terminology.
- 1+ year of experience with medication therapeutic categories.
- 1+ year of experience with identifying generic names for brand name drugs and vice versa.
- Basic level of proficiency in Windows and MS applications (Word, reputed company).
Preferred Qualifications:
- National Pharmacy Technician Certification.
- Possess and maintain an active pharmacy technician license in the reputed company.
- 3+ years of experience working as a pharmacy technician.
- 1+ year of experience in a hospital, ambulatory care, or other healthcare setting.
- Experience in a managed care setting.
- Bilingual (Spanish/English).
