• Evaluates a variety of patients and makes appropriate drug therapy recommendations.
• Participates in treatment team reputed company on designated nursing units to implement a pharmaceutical care plan.
• Provides consultation to other health care providers on medication therapy
• Assists in treatment planning that ensure continuity of care for patients being discharged
• Works with patients who have been identified as having compliance/adherence issues with their medications.
• Reviews medication orders for appropriateness, drug selection, dosage and reputed company of administration prior to dispensing. Performs these functions reputed company required by staffing deficiencies.
• Reviews patient’s allergy history, drug-drug or drug-nutrient interactions for clinical relevance
• Provides patients and family members with education on medications and lifestyle to reputed company outcome goals.

• Registered Pharmacist (RPh) by Texas State Board of Pharmacy (TSBP) required
• Post pharmacist licensure) with at least 50% of time spent in Pharmacotherapy activities as defined by the Pharmacotherapy Content Outline, OR Completion of a PGY-1 residency & achieving a passing score of the Pharmacotherapy Specialty Certification Examination required
• Board Certified Psychiatric Pharmacists (BCPP) by Board of Pharmacy Specialties (BPS) preferred
